Services We Provide

Social Workers provide specialist expertise in addressing the psycho-social aspects of: 

• The impact of disability
• Abuse, neglect, and family violence 
• Mental health, including carer issues of chronic sorrow and depression associated with grief and loss
• Housing, homelessness and/or inappropriate accommodation. 
• Individual and family adjustment to diagnosis

As a trained therapist, we want to assist you in enhancing your quality of life.   

We are trained in multiple therapeutic modalities (CBT, DBT, Single-Session Therapy) and can assist you and your family. 

Social Workers work therapeutically with individuals by:  

• Assisting with skill development, especially emotion regulation, distress tolerance and interpersonal effectiveness skills. 
• Addressing and resolving traumatic experiences and crisis. 

NDIS recognized assessment tools including the Psychosocial Assessment, WHODAS, DASS, ABAS-3 and LSP-16 are used and all of this information is included in the report we write at the end of the assessment to outline the details of your support and funding needs.  

Hopeful Horizons Therapy currently offers services to NDIS participants who are self-managed or plan-managed.
